Yes. To save time, go ahead and download these patient forms and bring them with you to the appointment.
What are some reasons for getting braces?
  • TOP FRONT TEETH PROTRUDE OR ARE "BUCKED"
This type of bite can result from the upper teeth being too far forward or the lower teeth being too far back. Function and aesthetics are affected with this bite.

  • OVERBITE
Top front teeth cover more than 25% of the bottom front teeth when the back teeth are biting together. Many times the lower front teeth bite into the roof of the mouth.

  • CROSS BITE
Top front teeth grow in behind the bottom front teeth. Often times we see unfavorable jaw growth and excessive tooth wear with untreated cross bites.

  • OPEN BITE
The upper and lower front teeth do not overlap. We often see habits and tongue thrusting with this bite. Proper chewing is definitely affected.

  • CROWDED, OVERLAPPED, MISPLACED, OR EXTRA TEETH
This occurs when teeth do not have enough room to erupt. Appropriate expansion can correct crowding, most times without tooth removal.

  • SPACING
Excessive spaces between teeth that persist after the top permanent canine teeth appear.

  • DENTAL MIDLINE NOT MATCHED
The centers of the top and bottom front teeth don't line up. This usually indicates that the back bite does not match properly.

How does Doctor decide the best treatment plan for me?
Asking questions about lifestyle and answering your concerns, Dr. Liz and her team can determine a course of treatment that will FIT into your daily life. Communication is the key to successful treatment and a beautiful result.
How much do braces cost?
The cost depends on the corrections required and the type of braces (traditional, invisible, invisalign) chosen. Again, all options will be discussed and the Doctor will help create a treatment plan that you will be able to live with financially. Our office offers several payment plans and will help with your insurance paperwork.
Do you take insurance?
Most dental insurance plans will cover a portion of your orthodonture treatment. Our staff will help you with the paperwork to get reimbursement for your out of pocket expenses. We do not accept assignment of benefits.
